All Forms can be uploaded below or emailed to info@apesf.org.
If your student attended a public school and is now switching to a private school, download the form below to be filled out by the public school. You can also use a report card that shows enrollment for at least 1 semester of the previous school year.
Public School Attendance Verification Form
If your student has received a prior award from another STO in the previous school year, download the form below.Previous STO Award Verification Form
If your student was ever in the Arizona Foster Care System, please download and fill out the following form.
Displaced Verification Form
If your student:
- Has and IEP, MET, or a 504 Plan from an Arizona public or charter school, please upload the paperwork here.
- Was previously on ESA but the ESA account has been closed, please upload your ESA CLOSURE LETTER.
- Attended school out-of-state or country, please provide a copy of their MOST RECENT REPORT CARD from the out-of-state or country school.
- Was homeschooled please provide one of the following: AFFIDAVIT OF INTENT TO HOMESCHOOL filed with your County School Superintendent, HOMESCHOOL WITHDRAWAL from your County School Superintendent, or PROOF OR PURCHASE of homeschool curriculum.
